Qualifications We need an RN who has at least two years of experience as a Clinical Documentation Integrity RN in the acute care inpatient setting with experience in the adult patient population. The candidate must have experience collaborating with clinicians, coding staff, and CDI leaders on medical records/charting to improve data integrity. Responsibilities The Clinical Documentation Specialist (CDS) is responsible for performing concurrent reviews of medical records to ensure complete and accurate clinical documentation to support severity of illness, risk of mortality for outcomes reporting, and hospital reimbursement. The position requires strong attention to detail, process orientation, and a global view of documentation requirements. The CDS must communicate effectively in oral and written form to physicians and other clinicians to promote accurate and complete documentation during the patient’s course of care, and support the hospital’s overall compliance efforts. Daily Duties Conduct reviews of designated medical records to facilitate the creation of a health record that accurately represents the acuity of the patient’s illness and the resources used to treat the patient by ensuring provider documentation supports all applicable diagnosis codes. Ensure assignment of the working DRG is supported by diagnostic and procedural findings from the medical record, including identification of secondary diagnoses that may result in complications and comorbidities (CC) or major complications and comorbidities (MCC). Utilize a compliant query process per guidelines and policies and follow each query through closure, documenting follow‑up and communication. Maintain integrity and compliance of medical record reviews, CDI documentation, and queries. Use UnityPoint Health’s selected CDI software and processes efficiently and effectively (e.g., CDI worksheet and workflow, physician query, query and DRG reconciliation). Meet the minimum productivity and quality assurance standards established by the CDI department. Collaborate with coding staff and leaders to improve knowledge and understanding of coding guidelines. Collaborate with physicians and other hospital staff to coordinate work and resolve issues. Collaborate with CDI leaders for feedback and guidance. Collaborate with other hospital departments as needed and directed by CDI leaders. Job Type & Location Contract position based out of West Des Moines, IA.
