We are looking for a reliable, empathetic, and attentive Hospice Aide to deliver essential personal care and emotional support to patients wherever they call home—be it a private residence, assisted living facility, or inpatient unit. This is a full-time position with weekdays from Monday to Friday, 8 am to 5 pm, and no weekends are required. You will be caring for patients in the Fox Valley area.

Your Role as a Hospice Aide:

• Deliver direct personal care to patients with terminal illnesses.
• Assist with personal hygiene, grooming, and toileting needs.
• Support safe mobility through the use of walkers and wheelchairs.
• Help with meal preparation, feeding, and light housekeeping tasks.
• Observe and communicate changes in the patients' physical or mental condition.
• Provide emotional and psychological support to both patients and their families.
• Maintain timely and accurate documentation of visits.
• Participate in team meetings and engage in Quality Improvement (QI) activities.
• Adhere to infection control practices and safety protocols consistently.

About You:

Qualifications:

• Active CNA license in the state of employment (Required).
• Current CPR certification (Required).
• Valid driver’s license and a reliable vehicle with insurance (Required).
• Six months or more of direct patient care experience (Preferred).
• Comfortable working in home settings, hospice facilities, or long-term care environments.
• Exhibit compassion, patience, and the ability to manage emotionally challenging situations.
• Willingness to work independently and adapt to changing patient needs.

Preferred Background (Not Required):

• Experience in home health, hospice, palliative care, or oncology settings.
• Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes.
• Familiarity with end-of-life care, wound care, or admissions support.
