Personal Care Aides and Certified Home Health Aides

JOB DUTIES / KNOWLEDGE

1. Personal care duties including bathing, grooming, feeding, turning and positioning.
2. Assist the patient with transfer, ambulating, prescribed exercise, and instructed by the Home Care Nurse.
3. When instructed by the Home Care Nurse, record weight, measure food and fluid intake and output.
4. Prepare modified diet as ordered by the home care nurse.
5. Observe and report specific vital signs and symptoms as instructed by Home Care
6. Remind the patient to take medications specifically ordered by the physician, which is ordinarily self-
7. Monitor skin changes daily, monitor changes and report to RN.
8. Perform incidental household functions necessary to maintain a safe E.g. comfort, cleanliness of the patient, and grocery shopping.
9. Report changes in the patients' physical status to the Home Care Nurse or Home Care Coordinator.
10. Submit annual mandatory compliance documents such as annual physical, PPD, drug screen, a copy of x-ray report (if positive PPD) and annual TB questionnaire.
11. Complete 12 hours of in-service every year.

QUALIFICATIONS

1. Have completed a training program approved by the New York State Department of Health as a Home Health Aide or be enrolled as a trainee aide in New York State approved training program under the supervision of qualified RN.
2. Be a minimum of 18 years of age, able to read and write English and to carry out
3. Have had a physical examination satisfying the New York State Department of Health requirements for Home Health Care Workers.

Pay: Up to $21.00 per hour

• Day shift
• Evening shift
• Holidays
• Monday to Friday
• Morning shift
• Night shift
• No nights
• No weekends
• Overnight shift
• Overtime
• Weekends as needed
• Weekends only

License/Certification:

• PCA or HHA Certification (Preferred)
• We also provide Free PCA Training
