  Patient Care Coordinator - Health Coach:

• We are seeking a Patient Care Coordinator - LVN/LPN or RN to become a part of our team!
• This position would be 20 hours per week initially and full time within 90 days.
• You will coordinate patient care and coaching programs with remotely monitored patients.
• These programs focuses on patient care and compliance as well as diet and exercise.
• This is a 100% work from home position. Anywhere in the USA.

Responsibilities:

• Communicate care information from programs.
• Engage new patients enrolled.
• Help patients track and report changes in condition.
• Update medical record systems with condition changes.
• Assist patients with healthcare questions and concerns.
• Help patients achieve goals.
• Get patients additional care from their providers as needed.

Qualifications:

• Previous experience in healthcare or other medical fields.
• Experience in remote health coaching or patient management is an asset.
• Experience in a patient-facing role.
• Previous medical clinic and work from home experience is an asset.
• Current Nursing license.
• Strong interpersonal qualities.
• Strong organizational and time management ability.
• Strong technical skills.
• Excellent written and verbal communication skills.
• Understanding of medical record systems.
