As we expand our clinical operations to support a growing patient and client base, we are seeking full-time medical assistants (MAs) to join our remote care team. In this role, you will support remote physiological monitoring (RPM) , contribute to the development and oversight of individualized patient care plans, and provide ongoing guidance and support to our patients.

Responsibilities:

• Monitor and review patient vitals and care plans using our proprietary technology platform
  
• Collaborate with registered nurse (RN) case managers to support the development and execution of care plans addressing clinical, behavioral, and social health needs
• Respond to alerts related to patient vitals and provide appropriate clinical guidance
• Deliver patient education and support through video, phone, and asynchronous communication
• Coordinate care with physicians and serve as a liaison between providers and patients
• Help patients build healthier habits through personalized coaching and support
• Coordinate care and follow-up with providers as needed

Required Qualifications:

• Active and valid medical assistant (MA) certification
• Bachelor's degree in nursing or a closely related healthcare discipline
• Strong customer service and communication skills
• Ability to work independently in a remote, technology-driven environment

Preferred Qualifications:

• Experience in case management
• Bilingual or multilingual proficiency
