\n\nThe Registered Nurse Case Manager is responsible for coordinating and delivering comprehensive nursing care to patients within an assigned territory. This role focuses on ensuring high-quality, cost-effective care while promoting patient independence and satisfaction. The RN collaborates with physicians, interdisciplinary team members, and community resources to support optimal health outcomes and smooth transitions of care.
